The undersigned is informed and aware of the rights due to him pursuant to Legislative Decree 196/2003 and art. 13 EU Regulation n. 2016/679 (hereinafter, “GDPR”) (Privacy Code) authorizes the Italian Society of Neurology, as well as any third company appointed by the Company to publish and disseminate the intervention, to process and communicate its personal data collected on the occasion of the realization of the initiative mentioned in the introduction and to use the same deeds and / or for the initiatives that should be achieved.
